It is the patient’s responsibility to understand their insurance coverage. We recommend contacting your insurance company to be sure your provider is in the network. Ultimately, the patient is responsible for paying for the services rendered. At present, we can accommodate patients covered by:

• Blue Cross Blue Shield (commercial and PMAP)
• Tricare West
• Health Partners (commercial and PMAP)
• UCare (commercial and PMAP)
• Cigna
• Medica/UBH/ UMR (commercial and PMAP)

We also accept cash payments.
However, we are not accepting patients with Medicare or State Medical Assistance.
